Weekend CRASH School Physical Examination Form
Each participant must have a routine physical. We must have a signed statement from a doctor stating that you are medically cleared to participate in the program.
Below is a link to a Physical Exam Form (your doctor may choose to use a form that is similar.) This form must be completed. Without this documentation, you will not be allowed to participate in the program (NO EXCEPTIONS.) These forms must be in our possession 2 WEEKS prior to your scheduled weekend. You may fax or mail them to us.
