Immunizations in Chittenden County

Our Immunization Public Health Nurse works closely with local families and medical practices to increase immunization rates in Chittenden County for infants, children, adolescents and adults.

The Burlington District Office offers a monthly immunization clinic to those under 65 without a medical provider or insurance and/or if their provider does not carry vaccines. The clinic is offered on the first Wednesday of each month from 2:00-5:30 p.m. To schedule an appointment or learn more, please call 802-657-4232.

Working with Health Care Providers

We assist local medical practices in increasing immunization rates by using the Vermont Immunization Program’s quality improvement model, known as AFIX. A public health nurse reviews practices’ rates, using data from Vermont’s Immunization Registry, and assesses current immunization services and processes. The model encourages practices to implement evidence-based strategies to improve services, decrease missed opportunities, and improve data quality.

In April 2016, pediatric practice Hagan, Rinehart, and Connolly was recognized by the Vermont Immunization Program for meeting the Healthy People 2020 goal of high childhood immunization coverage. Congratulations!
