DNR & COLST Orders

Beginning July 1, 2011, all Do Not Resuscitate (DNR) orders and Clinician Orders for Life Sustaining Treatment (COLST) for patients who are not admitted to a health care facility or residential care facility – or who are leaving or transferring from such a facility – must be issued on the form promulgated by the Vermont Department of Health.

The DNR/COLST form (PDF) is a single form with instructions on the first page.

The Health Department will begin rulemaking later this year to be effective March 1, 2012 to specify DNR identification and also may consider amendments to the to DNR/COLST form in that rulemaking process.

The Health Department will also be convening a study committee to make recommendations for rules concerning individuals who may give informed consent for a DNR/COLST order, but who are not the patient, the patient’s agent appointed in an advance directive, or the patient’s guardian.

These actions are required by 2011 Hospice and Palliative Care Act (Act 60). Act 60, adopted by the General Assembly in the 2011 session, includes provisions relating to Do Not Resuscitate (DNR) Orders and Clinician Orders for Life Sustaining Treatment (COLST).
