Blueprint Partnerships
Vermont Chronic Care Collaborative
The Vermont Chronic Care Collaborative (V3C) is an initiative of the Vermont Program for Quality in Health Care (VPQHC) and the Department of Health to improve care to Vermonters with diabetes and related cardiac disease. Fourteen teams from primary care and family practice sites from nine Vermont counties participated.
Improvements in care at each team's practice site will be realized through implementation of the Chronic Care Model, a model of care designed specifically for people with chronic illness.
