Work Plans
- Statewide Provider Practice - Goal: The proportion of patients receiving care consistent with evidence based standards will increase. 75 percent of PCP's in the state will adhere to diabetic evidence based care standards, and will use Vermont Health Record or its equivalent (by 2010).
- Information Technology - Goal: Vermont will have a Chronic Care Information System(CCIS) that supports statewide implementation of the Blueprint for both individual and population-based care management.
- Self Management - Goal: Vermonters with chronic conditions will become effective managers of their own health. By March 2008, increase the percentage of Vermonters age 18+ with diabetes who receive formal diabetes education from 45 percent in 2003 to 60 percent. By 2008, increase to 97 percent the proportion of people with diabetes in care who measure their blood sugar at least once per week.
- Community - Goal: Vermonters will live in communities that support healthy life-styles and offer opportunities to prevent and manage chronic conditions. Increase physical activity by 15 percent. Populate 211 with physical activity and nutrition programs by community with HSA's.
