Join Ladies First
How it Works
- Call the Ladies First enrollment counselor
Call "Kate" at 1-800-508-2222 or TDD 1-800-319-3141, or contact a local outreach worker - You can also download an application from our Ladies First Home Page. Foreign language and other formats available.
- Sign and return the forms (Consent and Enrollment) that Kate will send you, or that you downloaded from the web.
- Receive your Ladies First Membership and Screening cards in the mail. Then call a Ladies First doctor or nurse in your area (Kate will help you find one).
- Bring Membership and Screening cards to get a health screening.
Screenings include:- In-office breast exam
- Breast self-exam instruction
- Screening mammogram
- Pelvic exam
- Pap test
- Blood pressure*
- Total cholesterol*
- Body Mass Index (BMI)*
- Blood sugar test*
*only for members 40+; one time screening
- Use "Mammogram Prescription" to get a mammogram (prescription is attached to the Breast & Cervical screeing card).
- Receive test results. If follow-up or treatment is needed, Ladies First will help. The Provider office notifies Ladies First.
- Receive mailed, tailored self-help materials.
- Re-enroll and get screened again.
- Mammogram yearly age 40 and over
- Pap test every 2 years or as recommeded by your provider
- Cholesterol and Blood Pressure screening at least once
Eligibility

Services are available to:
- Vermont women Age 40 years or older
- Without Medicaid, VHAP, Primary Care Plus,or Medicare Part B
- With limited incomes (for example, in 2007 family income for two people can be up to $34,225/year).
Services are also available to:

- Without Medicaid, VHAP, Primary Care Plus, or Medicare Part B
- With limited income (for example, in 2007 the family income for two people can be up to $34,225/year).
- If you have breast symptoms or an abnormal Pap test, you are eligible for a limited membership. This includes breast and cervical screening and diagnostic tests, but not screenings for heart disease risk factors.
In addition, all women who have been screened through Ladies First and need treatment for breast and cervical cancer may be eligible for full Medicaid benefits during treatment, including coverage for pre-malignant conditions.
