Share your information

Recording and making your end-of-life and other critical healthcare instructions known is the first step toward having your health care wishes honored. Sharing them helps to provide others with the opportunity to discuss and understand your wishes.

Share with your agent

Share your wallet card registration ID number with your agent in the event you become unable to make healthcare decisions.

Let your agent know of any updates to your contact information, such as a change of address.

As a registrant, you are responsible for ensuring that your ID number is only shared with people you want to have access to information in the Registry.

Share with healthcare providers

Sharing your registration ID number with physicians and healthcare facilities is the best way for them to access your documents; it allows for quick and direct access to your documents when needed.
