Report Card Toolkit for Vermont Hospitals

Hospital Report Card

Act 53 of 2003 (18 V.S.A. § 9405b) requires hospitals in Vermont to produce annual hospital community reports, also known as Hospital Report Cards, reflecting the hospital’s performance in relation to quality, patient safety, hospital-acquired infections, nurse staffing, and pricing and measures of financial health.

For 2015: Hospitals required to have this information available on their websites must do so by May 29, 2015.


The Reporting Manual and template files are provided to assist hospitals in establishing their Hospital Report Card websites.
