State Health Assessment Plan
With the publication of Healthy Vermonters 2020, we begin our third decade of measuring the health of Vermonters. This is the state health assessment plan that documents the health status of Vermonters and will guide the work of public health through 2020.
This report presents more than 100 public health indicators and goals carefully chosen by state government, health and human services professionals, and the public from the many hundreds set out by the Healthy People 2020 initiative to improve the health of the nation.
