About the System
The Patient Safety Surveillance and Improvement System is administered by the Vermont Department of Health. It is designed to improve patient safety, eliminate adverse events, and support quality improvement efforts by Vermont hospitals.
Each Vermont hospital must report to the system any incidence of any of the National Quality Forum’s twenty-eight (28) serious reportable events.
To File a Complaint
The Patient Safety Surveillance and Improvement System is designed to accept reports of adverse events from hospitals, not from patients
