Patient Safety Surveillance and Improvement
- Hospital Reporting
The Patient Safety Surveillance and Improvement System is administered by the Vermont Department of Health for the purpose of improving patient safety, eliminating adverse events, and supporting quality improvement efforts by Vermont hospitals.
Each hospital must report the incidence of a National Quality Forum (NFQ) serious reportable event, and the occurrence of an Intentional Unsafe Act to Patient Safety.

Electronic Reporting: NQF Event
The Department of Health contracts with Quantros, Inc. to provide a secure electronic system for the reporting of a NQF event. Hospitals may submit electronic reports any time, seven day per week.
Paper Based Reporting: NFQ Event or Intentional Unsafe Act
Hospitals may report a NFQ Event or Intentional Unsafe Act by printing the applicable form and faxing or mailing to Patient Safety at the Health Department.
