An advance directive is what many people think of as a living will or a durable power of attorney for health care. It is a written document that outlines your wishes for medical treatment in the future, including if you are no longer able to make those decisions.

This section will help you to create and register or make changes to your advance directive. Health care professionals can also learn more about getting authorized or logging into the registry and Do Not Resuscitate orders.

If you already have a directive and would like to access your account with the Vermont Advance Directive Registry to view your document, you can log in to your personal account here.

Health care professionals can also learn more about applying for authorized access and Do Not Resuscitate orders. If you are a health care professional who already has authorized access, you can log in to your provider account here.
