Immunization Records in Vermont
The Vermont Immunization Registry (IMR) is a secure, centralized health information system containing Vermonters' immunization records. Medical providers and school nurses use the IMR to find patient histories, print IMR reports and report coverage.

Immunization Registry Login and System Information
As of September 2024, any new passwords must contain:
- a minimum of 14 characters
- numbers
- a combination of uppercase and lowercase letters
- special characters (& * % @ ...)
- and cannot include the user's first or last name

Immunization Registry Test System

Some users of the Vermont Immunization Registry will notice significant changes and updates to two practice-level reports, Vaccines Due by Practice and the Immunization Quality Improvement (IQIP) Report. Please review the guidance below for more information:
There are currently several issues in our vaccine forecaster logic that may incorrectly evaluate a patient’s vaccine history.
- What: Patients' evaluations of up-to-date status may be listed incorrectly or different than expected. Affected vaccine groups include: COVID (2024/2025 Season) and RSV.
- Why: This is due to a delayed release of an update to the vaccine forecaster logic that has not yet been implemented nationally.
- When will this be fixed? We expect to see progress made within the next month or two.
- Workaround: To evaluate a patient's vaccine schedule, refer to the current CDC Vaccine Schedules, which include drop-down information for expected vaccine groups and unique situations.
